Can anyone put the following paragraph into totally different words still making sense?

Each patient has their own dental record, so that when they have anther appointment the dentist can see the recorded results of their teeth and what had been done to them. All staff have meetings where the results are communicated in the dental surgery. Results can be reported over the phone or via email, but it is compulsory that the patients name is not given due to confidentiality.Can anyone put the following paragraph into totally different words still making sense?
Fred has a written record of what dental procedures have been done on him. Maria also has a written record of what dental procedures have been done on her. This helps remind the dentist what procedures have been done on each patient he sees. This also helps in case a different dentist sees a patient.





The entire staff meets either via e-mail or over the phone to review what procedures are done and what the results were.





The specific names of the patients (probably just the patients age an procedure done) are not given, just the procedure performed and it's result.
